Medical Records
Spine & Orthopedic Center has partnered with ChartRequest to make requesting medical records simple and convenient. Patients, healthcare providers, attorneys, and other authorized parties can submit requests, track their status, and receive records online.
For Patients
- Select the Request Medical Records button and choose "I am a Patient."
- Create a free ChartRequest account.
- Enter the patient's information and submit your request.
- You will receive an email when your records are ready to download.
You may request your own records or records for a dependent. Patients can also request completed forms, such as disability, FMLA, and school forms, through the same portal.
For Healthcare Providers
Select "I am a Healthcare Provider" to request or exchange records for continuity of care. You can also use the portal to securely upload records to our team.
For Attorneys & Other Authorized Parties
Select "I am a Third Party Professional" to request records on behalf of your client. A valid authorization signed by the patient or their legal representative is required.
Processing Time & Fees
Most requests are fulfilled within 7 to 10 business days. A rush option is available within the portal. Any applicable fees are calculated automatically in accordance with Florida and federal guidelines and will be displayed before you complete your request.
