Now that your appointment is scheduled, please check your email for an invitation from Spine & Orthopedic Center to sign up in your Patient Portal called Breeze. Upon opening your email invitation, please click on the Let’s Get Started button to create your Patient Portal account.
Once your account is created in Breeze, click on the MENU icon on the top left side of the page, and select FORMS. Select Telemedicine Consent Form and sign it. If the consent form isn’t signed prior to the appointment and/or a representative from our practice was not able to reach you, please contact the office to reschedule the appointment or expect a call from our office to do so.
Once you have successfully signed up for the patient portal, you’ll receive another email and a text message with your telemedicine appointment confirmation. Follow the email instructions to join your virtual consultation on the set appointment date and time. We use Carecloud Live for our telemedicine appointments and you can access it via computer, smartphone or tablet.
If you experience technical difficulties when trying to join your consultation, please send us a detailed message by filling out the Telemedicine Support form in this page (exclusively for telemedicine patients) to let us the know the issue. One of our representatives will call you as soon as possible to reschedule your appointment.
A $50 cancellation fee may apply if the appointment is not cancelled at least 24 hours in advance.
